Senior Activity Approval Form
2025-2026

Rationale: Each Senior interested in participating in one or all of the following Senior Activities must acquire the signatures of Ms. Cobb, their Guidance Counselor, and AP Lafergola before their participation is approved. Ms. Cobb and AP Lafergola will attest to the student’s attendance, academic and behavioral standing.

Directions: Signatures should be placed in the box under the activity. Students should bring the completed form to Ms. Chantel Dawson in the Main Office (room 128)  before school, during lunch, or at the end of the school day.
